STEP 1: Fill out the Membership Application and sign the Conditions of Membership forms

STEP 2: Attach a photo of your government-issued photo ID along with proof of your condition or Medical Document (i.e., prescription)

STEP 3: Submit forms for review

STEP 4: Receive a menu and payment instructions
