How Do I Get A Cannabis Card?
In order to obtain a cannabis card, begin by consulting a health practitioner. Authorized health practitioners include family physicians and nurse practitioners. Either of these practitioners in charge of your care can provide a consultation in provinces and territories that support medical use of marijuana.
The next step is for your health practitioner to draft and sign a medical document. This document must include the following:
- your full name;
- your date of birth;
- address where you consulted with the practitioner;
- daily grams of dried marijuana you are authorized to use and the period of use (i.e. number of days, weeks or months)
- the healthcare practitioner’s name and, administrative information including profession, license number, province of license, business address, phone number, fax number
- a verification statement by the healthcare practitioner that the information contained in the document is factually correct and complete.
The document must be signed and dated by the health practitioner. Of importance, the period of use cannot exceed one year from the date the medical document was signed by the healthcare practitioner.
You can now register with a licensed producer of your choice. The licensed producer will require the original medical document. This can be achieved by secure fax from your health practitioner or you can provide it directly. Contact the licensed producer for further application requirements.
Once registered, you will be able to purchase marijuana from the licensed producer. In your first shipment, you will be provided with official documentation from your licensed producer. In lieu of this documentation, the label from your prescription may also be presented as a “card”, should local authorities ask for proof of permission to possess cannabis.
